Tim Morrison, LCSW, ACM
Director, Care Coordination
As the Director of Care Coordination, Tim Morrison oversees the Departments of Case Management, Social Work, and Palliative Care. He is responsible for the delivery of coordinated, interdisciplinary care planning, efficient and safe discharge planning, integrated palliative care, and clinical social work services. All of these areas work to optimize the patient-centered plan of care by removing system and psychosocial barriers to increase access to care, improve outcomes and insure the appropriate use of resources.
Prior to joining SHC, Tim was the Care Coordination Manager for Surgery and Transplant at Ronald Reagan UCLA Medical Center. He also held clinical instructor faculty appointments in the UCLA School of Public Affairs and the UCLA David Geffen School of Medicine.
Tim is a Licensed Clinical Social Worker and Accredited Case Manager. He has a Bachelor of Music degree from the University of Miami and a Master of Social Welfare degree from UCLA.
