    Confluent or focal periventricular white matter hyperintensities on T2-weighted MR images was found in 75% (15/20) of elderly demented patients who met NINCDS-ADRDA criteria for probable Alzheimer’s disease. Sulcal enlargement was significantly associated with confluent and focal alterations, but MR findings failed to distinguish among clinical subtypes, implying the lact of diagnostic or clinical significance for these white matter alterations on MR in this population. Periventricular white matter hyperintensity, whether reflecting Alzheimer’s pathology or a nonspecific finding unrelated to this disorder, need not dissuade the clinicial from a diagnosis of Alzheimer’s disease.
