To refer a new patient:
- Complete a Referral Form
- Gather supporting documentation including
- Recent/relevant typed clinical notes/test results (i.e. History & Physical, MRI/CT/X-rays results)
- Proof of Insurance
- Authorization information (if required)
- Fax referral form with supporting documentation to 650-320-9443
