The Medical Staff Services is a shared service and performs joint medical staff and credentialing functions for both Stanford Hospital & Clinics and Lucile Packard Children’s Hospital.Refer a Patient
To refer a new patient, fill out a Referral Form
and fax with supporting documentation to 650-320-9443.

The Stanford Transfer Center serves as a gateway for transfers from other facilities.
Physician Liaison Service
Our physician liaisons help disseminate information about new Stanford Hospital & Clinics programs, new faculty, and CME courses. They are also the referring provider’s direct link to Stanford Hospital & Clinics, helping to identify and resolve issues, and making sure that the referring providers’ experience is a positive one.
