Registration
To register for any of the Life Support Training Center classes:

complete the Educational Assistance form
if you are a Stanford employee and qualify for tuition or
transfer of department funds. Fax the Education Assistance
form to (650) 725-9937

If you are registering from outside of Stanford Hospital &
Clinics or do not qualify for Education Assistance at Stanford
Hospital & Clinics, please complete the Registration
form
If paying by check, please mail form to: Stanford Hospital & Clinics, 300 Pasteur Drive, Stanford, CA 94305.
If paying by credit card, either mail or fax form to (650) 725-9937.
