This is a program devoted to meeting the needs of aging adults and their families, and providing them a continuum of care with support and resources. Aging Adult Services is supported in part by the Ronald and Ann Williams Charitable Foundation, Julian N. Stern, President and Trustee.

In addition to the 6 programs listed above in the upper left column (Dementia Support, Strong for Life, Checking In, Advance Directives, Vial of Life, Lifeline), we also provide:

ments, physician referrals, advocacy, coordination of services, access to community resources and educational workshops.
- In-Patient Delirium Consult Service (650) 721-6014: Kathleen Turner-Hubbard, Geriatric NP.
- Geriatric Medicine Out-Patient Clinic & Consultation Service (650) 725-5222: Yusra Hussain, MD, Medical Director; Kathleen Turner-Hubbard, Geriatric Nurse Practitioner; and Mehrdad Ayati, MD.
- Geriatric Medicine In-Patient Consult Service, page 26888: We assist primary teams of any specialty to provide outstanding care for their elderly patients. Staff is available for consultation 7 days a week. This clinical team also partners with Aging Adult Services for comprehensive care post-discharge.
- Caregiver Support Program (650) 725-4137: This program provides one-on-one short-term telephone support and caregiver resources to family caregivers of Stanford patients to help them take care of themselves while caring for their loved ones.
- Chronic Disease Self Management Program (650) 725-4137: Classes about self-managing chronic diseases.
- Better Choices, Better Health: Based on Stanford University's Chronic Disease Self-Management Program, these online workshops are designed to improve your health and your life. Register online at selfmanage.org/signup.
