Welcome to Aging Adult Services (AAS) at Stanford!
This is a program devoted to meeting the needs of older adults and their families, and providing them a continuum of care with support and resources. AAS is supported in part by the Ronald and Ann Williams Charitable Foundation, Julian N. Stern, President and Trustee.
The AAS team, with elder care specialists and a medical director, is here to assist you. Our Aging Adult Services brochure provides information and resources related to aging and geriatric health. - Geriatric Health Services (GHS) (650) 723-1303: This program offers consultations, assessments, home visits, assistance with appoint ments, physician referrals, advocacy, coordination of services, access to community resources and educational workshops.
- Geriatric Out-Patient Clinic and Consultation Service (650) 387-6777:
Dr. Yusra Hussain, M.D., Medical Director.
- In-Patient Delirium Consult Service (650) 721-6014: Kathleen Turner-Hubbard, Geriatric Nurse Practitioner.
- Caregiver Support Program (650) 725-4137: This program provides one-on-one short-term telephone support, as well as caregiver resources, to family caregivers of Stanford patients to help them take care of themselves while caring for their loved ones.
- Chronic Disease Self Management Program (650) 725-4137: Classes about self-managing chronic diseases.
- Medical Assessment (650) 725-3647: FREE medication assessments in collaboration with the Department of Pharmacy at Stanford Hospital & Clinics. Assessments are done by a pharmacy intern and provide a review of all medications including prescription, over-the-counter and herbal therapies. The review is done at home or an independent living facility where the patient resides. The medication assessment consists of side-effect management, drug to drug and drug to food interactions.
