Ambulatory Care
- Alice Wen, Pharm.D. received her degree at University of Southern California in 1998 and completed an ambulatory care residency at Kaiser Orange County. She worked as internal medicine pharmacist at VA Palo Alto before coming to Stanford in 2002. She currently works as an ambulatory care pharmacist specializing in anticoagulation. She has also achieved National Certification as an Anticoagulation Care Provider (CACP).
- Rakhee Nishtala, Pharm.D. received her undergraduate degree in Molecular and Cell Biology from the University of California, Berkeley and her Doctor of Pharmacy degree from the University of California, San Diego. She completed her residency at Stanford Hospital and Clinics and currently works in various areas, including the OR Satellite Pharmacy and the Oral Anticoagulation Clinic.

Ya-Hwei Cheng, MS, Pharm.D. received her MS in health system management from Union University, NY and pharmacy degree from University of Florida. She joined Stanford Hospital and Clinics in 1997. She has practiced in medicine, critical care, cardiology, orthopedics, pediatrics, oncology and investigational drug services. She is currently a clinical pharmacist specializing in BMT.

Fu-ying Lee-Lam, R.Ph. received her B.S from St. John’s University, New York in 1988. She worked at Memorial Sloan Kettering for five years before moving to California. Currently she works at Stanford Hospital and Clinics as a Bone Marrow Transplant pharmacist. BMT has been her focus of practice for the last 15 years.

Joanna Wang, Pharm.D. received her undergraduate degree in Biology from the University of California, Irvine and her Doctor of Pharmacy degree from the University of Southern California in 1991. She completed her residency at University of California, Los Angeles in 1992 and currently works as a clinical pharmacist in Bone Marrow Transplant & Oncology.
Cardiology

Kelly Matsuda, Pharm.D. received his pharmacy degree from the University of the Pacific. He practiced for 2 years in Oncology and General Medicine and is currently a cardiology pharmacist.
- Manya Sarram, Pharm.D. (Lead Pharmacist) graduated in 1988 with a B.S. in pharmacy from the University of Munich, Germany. After moving to the United States, she went on to complete a 2-year Pharm.D/residency program at the University of Washington, Seattle. She has been a clinical pharmacist in Cardiology Medicine here at Stanford since 1995. Currently she is one of the lead pharmacists and is dividing her time between clinical and administrative duties.
Central Operations
- Erika Boetius, Pharm.D. Attended UC Davis for undergraduate years and then graduated from the University of Pacific in 1987. After working 11 years in retail, she was hired as the Pharmacist in Charge for the Stanford Outpatient Pharmacy in 1998. She worked 3 years in the Outpatient setting and then transferred into the Inpatient Pharmacy. She has worked as part of the management team and is currently primarily staffing the Central Clean Room and involved with the preparation of IV products.
Critical Care

April Fong, Pharm.D. received her undergraduate degree from UCLA and her Doctor of Pharmacy degree from the University of Washington in 2003. She completed her pharmacy practice residency and critical care residency at Stanford Hospital and Clinics. She currently works as a critical care pharmacist.

Fred Hurt, Pharm.D. (Cardiothoracic Surgery) graduated from University of the Pacific. He completed his pharmacy practice residency at VA San Diego and a critical care residency at Medical University of South Carolina. He is currently a clinical pharmacist working in the operating room pharmacy.

Janice Hsieh, Pharm.D. graduated from University of Southern California. She completed her general pharmacy practice residency at Kaiser Vallejo. She worked at Kaiser Redwood City for 2 years and joined Stanford's Department of Pharmacy as a critical care pharmacist in 2004.

Mark Klanjac, Pharm.D., (Lead Pharmacist) received his pharmacy degree from the University of the Pacific and completed his pharmacy practice residency at Stanford Hospital and Clinics in 2006. He currently works as a lead pharmacist overseeing the OR and ICU areas.

Lina Meng, Pharm.D., BCPS graduated from University of California, San Diego in 2009. She completed her pharmacy practice residency at the University of California, San Francisco in 2011 and a critical care residency at Stanford Hospital and Clinics in 2011. She currently works as a critical care pharmacist covering medical-surgical ICU and cardiology.

Sandra Leigh Bardas, BSP, FCSHP received her pharmacy degree from Massachusetts College of Pharmacy and completed an ASHP-accredited residency at Massachusetts General Hospital. She has practiced in various areas at Stanford Hospital and Clinics, including operating room pharmacy, investigational drug services, and critical care. She currently works in the Emergency Department Pharmacy.

Carolyn Nguyen, Pharm.D. graduated from University of the Pacific. She practiced for 2 years in Oncology/Hematology and BMT and currently works as a clinical pharmacist in the Emergency Department.

Latanya Dean, Pharm.D., (Lead Pharmacist) received her degree from the University of Washington in 2001. She completed a general practice residency at Stanford Hospital and Clinics in June 2002. After residency she accepted a clinical position at SHC and currently works in the Cancer Center as a Lead Pharmacist overseeing the inpatient and outpatient oncology pharmacy services.

She completed her pharmacy practice residency and PGY2 hematology/oncology pharmacy residency at Stanford Hospital and Clinics. Upon the completion of her PGY2 residency in 2010, she accepted a position at SHC as a clinical pharmacist specializing in hematology and oncology.

She completed her pharmacy practice residency at Shands Jacksonville/The University of Florida Academic Health Center and PGY2 oncology pharmacy residency at University of Washington Medical Center. She worked as a clinical oncology pharmacist at Hematology/Oncology Ambulatory Clinic at Kaiser Permanente and Sutter Health for seven years before joining the hematology/oncology specialty area at Stanford Hospital and Clinics.

Faye Tsung, Pharm.D. received her Bachelors in Pharmacy at University of Toronto in 1989. In 1990, she completed an Industrial Pharmacy Residency at Eli Lilly Canada Inc. In 1994 she received her Doctor of Pharmacy degree at Philadelphia College of Pharmacy and Science. The following year she completed an oncology residency at University of Illinois at Chicago. She joined Stanford Hospital and Clinics in 2002 and currently works as a BMT/Oncology pharmacist.

Emily Mui, Pharm.D., BCPS received her pharmacy degree from Northeastern University in 2008. She completed her pharmacy practice residency and infectious disease speciality residency at Brigham and Women's Hospital in 2010. She works as a clinical pharmacist in general medicine.

Denise Gin Bragg, Pharm.D., BCPS graduated from University of California, San Francisco and completed her pharmacy practice residency at University of California, San Diego and a critical care residency at Stanford Hospital and Clinics. She is currently a clinical pharmacist specializing in critical care medicine and internal medicine.
- Sean Carlton, Pharm.D., BCPS received his degree from Virginia Commonwealth University in 2009. He completed a general practice residency at Stanford Hospital and Clinics in 2010. His primary areas of practice are Internal Medicine, Emergency Medicine, and IV preparation.
Practice Management
- Michael Brown, Pharm.D, Director of Pharmacy received his undergraduate degree in physiology from the University of California at Davis and his Doctor of Pharmacy degree from the University of the Pacific, School of Pharmacy. Following the completion of his Pharm.D. Degree, he completed an A.S.H.P. accredited residency at the University of California at San Francisco. Prior to joining Stanford Hospital and Clinics, he held Director of Pharmacy positions at American River Hospital in Carmichael, CA, and San Ramon Regional Medical Center in San Ramon, CA. He was the General Manager for the Northern California Home Infusion Center for Apria Healthcare in Hayward, CA, and immediately prior to joining Stanford was a Senior Director of Operations for the Nuclear Pharmacy Division of Cardinal Health.
- Jeff Inokuchi, Pharm.D, Assistant Director of Pharmacy received his BS in cellular biology from UC Davis, PharmD from UC San Francisco, and completed a residency in Hospital and Clinical Pharmacy at UCSF in 1981. He has worked at Stanford since 1982, clinically in critical care, surgery, transplant and oncology. Current responsibilities include computer, distribution, and clinical systems, drug contracts, and cost accounting.
- Dana K. Radman, Pharm.D., BCPS, Manager of Clinical Effectiveness received dual undergraduate degrees in Animal Physiology/Neuroscience and Psychology from the University of California, San Diego. She received her pharmacy degree from the University of Southern California in 2006. She subsequently completed a pharmacy practice residency at Stanford Hospitals and Clinics in 2007 and worked as a clinical pharmacist before transitioning into management in 2010. She currently is the Secretary for the Pharmacy and Therapeutics Committee and serves in a management role while continuing to practice as a clinical pharmacist.
- Deepak R. Sisodiya, Pharm.D., BCPS, Manager – Professional Development received his BS in Pharmaceutical Sciences from the University of Alberta and his PharmD from the University of Michigan in 2002. After completing a pharmacy practice residency at Stanford Hospital and Clinics he worked as a clinical pharmacist in general medicine with an interest in infectious diseases. In 2010 he made the move into his management position which includes overseeing all three residency programs.
Psychiatry
- Julie Newton, Pharm.D. (Lead Pharmacist) graduated from University of the Pacific in 1985. After working 3 years for a small independent pharmacy, she completed a general residency at VA Long Beach. She has worked for Stanford since 1989 and has staffed in several pharmacy coverage areas including the OR, pediatric ICU, and skilled nursing. In her current position she continues to work with the in-patient neurosurgery, cardiac step-down, ENT, urology, and psychiatry patient populations as well as is the Lead Pharmacist for the Central Pharmacy, IV room, B2 satellite pharmacy & ED pharmacy satellite areas. She continues to precept pharmacy residents and students along with her colleagues in the B2 satellite pharmacy in the area of psychiatry.
Transplant
- Roy Lee, Pharm.D., BCPS received his undergraduate degree in Molecular, Cell & Developmental Biology from the University of California, Los Angeles and his Doctor of Pharmacy degree from the University of California, San Diego in 2009. He completed his residency at Stanford Hospital and Clinics in 2010 and currently works as the Heart, Lung & Heart-Lung Transplant pharmacist.
- Uerica Wang, Pharm.D., BCPS received her Pharm.D. and completed a pharmacy practice residency at the University of California, San Francisco. She currently works as a kidney, liver and intestinal transplant pharmacist at Stanford Hospital and Clinics.
