- Possess a Doctor of Pharmacy from an ACPE accredited school or college of pharmacy
- Participate in an on-site interview at Stanford Hospital & Clinics
- Obtain licensure to practice pharmacy in the state of California before September 1, 2012: http://pharmacy.ca.gov

Selected applicants will be required to visit Stanford Hospital & Clinics for an on-site interview in February 2012. For consideration, the online application and all supporting documentations must be received by January 4, 2012.
On-Site Interview
- After review of application materials by the residency committee, applicants will be contacted for a three hour on-site interview
