Preceptors Biographies
Michael Brown, Pharm.D, Director of Pharmacy (Administration) received his undergraduate degree in human physiology from the University of California at Davis and his Doctor of Pharmacy degree from the University of the Pacific, School of Pharmacy. Following the completion of his Pharm.D. degree, he completed an A.S.H.P. accredited residency at the University of California at San Francisco. Prior to joining Stanford, he was a Senior Director of Operations for the Nuclear Pharmacy Division of Cardinal Healthcare.
Jeff Inokuchi, Pharm.D, Assistant Director of Pharmacy (Administration) received his BS in cellular biology from UC Davis, PharmD from UC San Francisco, and completed a residency in Hospital and Clinical Pharmacy at UCSF in 1981. He has worked at Stanford since 1982, clinically in critical care, surgery, transplant and oncology. Current responsibilities include computer, distribution, and clinical systems, drug contracts, and cost accounting.
Arash Dabestani Pharm.D, MHA, Associate Director of Pharmacy (Administration) received his Doctor of Pharmacy degree from Campbell University in North Carolina. Following the completion of his Pharm.D. degree, he earned a masters in health administration from the University of North Carolina in Charlotte and completed a fellowship with the Advisory Board in Washington, D.C. Prior to joining Stanford, he was the Director of Pharmacy Operations for Potomac Hospital in Virginia.
Dana K. Radman, PharmD, BCPS, Manager of Clinical Effectiveness (Administration) received dual undergraduate degrees in Animal Physiology/Neuroscience and Psychology from the University of California, San Diego. She received her pharmacy degree from the University of Southern California in 2006. She subsequently completed a pharmacy practice residency at Stanford Hospitals and Clinics in 2007 and worked as a clinical pharmacist before transitioning into management in 2010. She currently is the Secretary for the Pharmacy and Therapeutics Committee and serves in a management role while continuing to practice as a clinical pharmacist.
Alice Wen, Pharm.D. (Ambulatory Care – Oral Anticoagulation Clinic) received her degree at University of Southern California in 1998 and completed an ambulatory care residency at Kaiser Orange County. She worked as a medicine pharmacist at VA Palo Alto before coming to Stanford in 2002. She currently works as an ambulatory care pharmacist.
Ann Vongspanich, Pharm.D. (Ambulatory Care – Oral Anticoagulation Clinic and Cancer Center) received her BS degree in biology at Santa Clara University then obtained her Pharmacy degree at UOP, pharmacy school. She finished her intern year at VA PaloAlto and completed her Pharmacy Practice Residency at Stanford in 2005. She currently works in ambulatory care clinic and investigational drug service.
Latanya Dean, Pharm.D. (Ambulatory Care –Cancer Center) received her degree from the University of Washington in 2001. She completed a general practice residency at Stanford in June 2002. After residency she accepted a clinical position at Stanford and currently works in the Cancer Center as an oncology clinical pharmacist.
Faye Tsung, Pharm.D. (Ambulatory Care –Cancer Center) received her degree at Philadelphia College of Pharmacy and Science. In 1995 she completed an oncology residency at University of Illinois at Chicago. She joined Stanford in 2002 and currently works as a BMT/Oncology pharmacist.
Fu-ying Lee-Lam, Pharm.D. (Bone Marrow Transplant) received her B.S from St. John's University, New York. She worked at Memorial Sloan Kettering during her internship and stayed for 7 years. She has been at Stanford since 1995 and her area of specialty is BMT and Oncology.
Joanna Wang, Pharm.D. (Bone Marrow Transplant) graduated from University of Southern California in 1991 and completed a residency at UCLA. She worked for 5 years at UCI medical center in the critical care unit. She began working at Stanford in 1997 and is currently the clinical pharmacist in BMT/Oncology.
Kelly Matsuda, Pharm.D. (Cardiology) is a clinical pharmacist specializing in Cardiovascular Medicine at Stanford. He received his degree from the University of the Pacific in 1982. He practiced for 2 years in Oncology and General Medicine and is currently a cardiology pharmacist.
Manya Sarram, Pharm.D. (Cardiology) graduated in 1988 from the University of Munich, Germany. She then went on to complete a 2-year Pharm.D./residency program at the University of Washington, Seattle. She has been a clinical pharmacist in Cardiology Medicine here at Stanford since 1995.
Mark Klanjac, Pharm.D. (Critical Care) received his B.S. degree in Pharmacology from the University of California at Santa Barbara and a B.S. degree in Biology with and emphasis in health sciences from the University of the Pacific. He earned his Pharm.D. degree from the University of the Pacific and completed a pharmacy practice residency at Stanford. He currently works mainly in operating room pharmacy.
Craig Sterling, Pharm.D. (Critical Care) received his degree at University of Southern California. He then completed a pharmacy practice residency at Ohio State. Currently he works at Stanford specializing in critical care.
Mike Schoshinski, Pharm.D. (Critical Care) received his degree at UCSF. He completed a pharmacy practice residency at Stanford Hospital and currently works as a critical care pharmacist.
Sandra Leigh Bardas BSP, FCSHP (Emergency Medicine) received her pharmacy degree from Massachusetts College of Pharmacy and did an ASHP accredited residency at Massachusetts General Hospital. She has practiced in various areas at Stanford Hospital including operating room pharmacy, investigational drug services and critical care. She currently works in the Emergency Department Pharmacy and OAC Clinic.
Carolyn Nguyen, Pharm.D. (Emergency Medicine) graduated from University of the Pacific in 2001. She practiced for 2 years in Oncology/Hematology and BMT and currently works as a clinical pharmacist in the Emergency Department.
Noriko Okada, Pharm.D., BCPS (Inpatient Consult) obtained her pharmacy degree at University of Maryland. She has held various positions including a clinical pharmacist at Stanford, a clinical educator and an assistant professor at a pharmacy school in Tokyo. She currently works as the Pharmacy Practice Residency Director.
Deepak Sisodiya, Pharm.D. (Internal Medicine) graduated from the University of Michigan. He completed a residency at Stanford and is interested in infectious disease. He has practiced internal medicine at Stanford since 2003.
Lawrence Witt, Pharm.D. (Internal Medicine) obtained his pharmacy degree at UCSF and stayed there to complete a pharmacy practice residency. He also completed an addition Pharmacy Residency at the University of Connecticut. His primary duties are with the Internal Medicine and Transplantation Services, but he has also worked in other areas including Hematology/Oncology and Cardiology.
Anne (Yi-an) Chen, Pharm.D., BCOP (Oncology/Hematology) received her pharmacy degree from the University of California at San Francisco in 2003 and completed her Pharmacy Practice Residency at Stanford Hospital and Clinics in 2004. She is currently a clinical pharmacist specializing in Hematology/Oncology.
Julie Newton, Pharm.D. (Psychiatry) graduated from University of the Pacific in 1985. After working 3 years for a small independent pharmacy, she completed a general residency at VA Long Beach. She has worked for Stanford for the past 15 years and has experience in the OR, pediatric ICU, and skilled nursing facilities. She is currently involved with inpatient psychiatry areas, neurosurgery, orthopedics and rehabilitation medicine.
Belinda Lim, Pharm.D. (Psychiatry) received her degree at University of Pacific and completed a residency at University of Cincinnati. She worked in Southern California and before coming to Stanford. She has been at Stanford since 2002 and is currently a psychiatry pharmacist.
Magdalena Cudny, Pharm.D., BCPS (Oncology/Hematology) received her degree from the University of California, San Francisco in 2006. She completed a general practice residency at Stanford Hospital and Clinics. She worked as a clinical pharmacist for 2 years before transitioning into the inpatient hematology and oncology specialty area at Stanford Hospital.
Uerica Wang, Pharm.D., BCPS (Solid Organ Transplant) received her Pharm.D. and completed a pharmacy practice residency at the University of California San Francisco. She currently works as a kidney and liver transplant pharmacist at Stanford.
Khanh Nguyen, Pharm.D. (Psychiatry) received her B.S degree in System Physiology with a minor in Chemistry in 1995. She briefly worked for a medical device company before going back to school to obtain her Pharm.D. from UC San Francisco. She is currently a clinical pharmacist covering inpatient psychiatry.
April Fong, Pharm.D. (Medical & Surgical ICU and General Surgery) received her degree at University of Washington. She completed a pharmacy practice residency at Stanford and currently works as a critical care pharmacist.
