Transfer Center Services
- Physician consultation, trauma patients and emergent transfers are accepted 24 hours a day.
- Urgent (nonemergent) transfers are managed on a case-by-case basis Monday through Friday, 8am-4pm
- Coordination of transportation arrangements including:
- Ground ambulance: BLS (Basic Life Support), ALS (Advanced Life Support), CCT (Critical Care Transport)
- Air ambulance: fixed wing and helicopter
- Specialty transport: IABP (Intra Aortic Balloon Pump), LVAD (Left Ventricular Assist Device), Critical Care and Trauma
- Referring hospital name, phone number, and fax number
- Referring physician name and contact numbers
- Patient name, age, diagnosis, and type of specialty care or service requested
- The clinical status of the patient
Transfer Center Process at SHC
- A hospital representative or physician calls the Transfer Center expressing interest in transferring a patient to Stanford Hospital & Clinics.
- The Communications Specialist at SHC Communications Center will locate the appropriate SHC physician and facilitate a recorded conference call between the referring and accepting doctors to determine a preliminary clinical evaluation.
- Admission of emergent patients accepted by a physician will be facilitated into SHC with the assistance of the Transfer Center nurse. The Transfer Center policies and guidelines are designed to adhere to EMTALA regulations and to facilitate the request in the fastest manner possible.
- Clinical presentation obtained via a detailed clinical screen.
- Level of care required, medical necessity, and benefit to patient.
- Evaluation of closer facilities offering the same required level of care.
- Insurance availability, eligibility, and authorization for transfer.
Immediate Response by Specialized Teams
Mode of transport is determined by sending physician request and can be adjusted based on patient acuity. When Stanford's medical team of specialized nurses is asked to respond, either by ground or by air ambulance, Stanford is extending its state-of-the-art clinical capabilities. Our specialized critical care teams are supported by patient care protocols derived from evidence-based medicine, and overseen by a board certified emergency medicine attending physician. This assures the physician, and family, that the immediate care will provided the best possible outcome. Remember, each nonemergent transfer is individually evaluated. However, demands can exceed capacity. Whenever possible, Stanford Transfer Center will offer guidance in finding a suitable alternative if we are unable to accept a request for transfer.
