Registration & Payment
Our helpful Patient Access Representatives are located in the
lobby. Upon arrival, you will meet with them for "Quick Reg,"
the first part of patient registration.Registration takes place in two parts. First, you will meet with the Patient Access Representative (Registrar) in the registration area at the rear of the waiting room for “Quick Reg” (registration), which takes only a few minutes.
You will be asked the following questions:
- Patient name
- Gender
- Date of birth
- Mailing address
- Name of primary care doctor for notification
- Chief complaint (medical reason[s] for your visit to the Emergency Department)
Photo identification will be requested at this time. If you have a Stanford Medical ID card, please present it during registration. We do not collect insurance information at this time.
After you have been seen by the doctor and have been assigned a bed, a Patient Access Representative in a blue blazer will come to your bedside to complete the registration process.
If you are insured, this process includes:
- Collecting your insurance (card) information
- Confirming eligibility with your insurance company
- Reviewing confidentiality, terms and conditions of services
- Reviewing and signing the HIPAA (Health Insurance Portability and Accountability Act) form
- Collecting co-pays and having the patient or responsible person sign a receipt for payment
- Answer any questions you may have for the Patient Access Representative
If you are uninsured, the following will occur:
- Review confidentiality, terms and conditions of services
- Review and sign the HIPAA (Health Insurance Portability and Accountability Act) form
- A Financial Counselor will be assigned to assist you or the responsible person in arranging for payment.
- Financial assistance forms are available in both English and Spanish
- Translation services are available
