Before coming to your scheduled appointment:
- Read the New Patient Letter
- Print and complete the New Patient Questionnaire
- If you are being referred to the dietitian for bariatric surgery counseling, print and complete the Bariatric Patient Questionnaire
The Stanford Department of Clinical Nutrition staffs registered dietitians to provide medical nutrition therapy for adult ambulatory care patients.
