What to expect from an appointment with the insomnia and behavioral sleep medicine program
A consultation appointment is an office visit with one of our behavioral sleep medicine providers. During the evaluation, your provider will conduct a clinical interview focused on the presenting sleep issue and work with you to develop a treatment plan.
It is always wise to prepare a list of questions and key issues you would like to discuss before your appointment to ensure all of your questions are answered.
What to do before your appointment:
In order to ensure your visit goes smoothly, please:
- Verify your insurance coverage and bring your current insurance card (see How Much Will This Cost section below)
- Complete a sleep history questionnaire before arriving at the Sleep Center
We encourage you to complete our on-line version of the questionnaire or you may bring a completed hard copy of this alternative questionnaire (PDF) to your appointment.
When to arrive:
Please arrive 15 minutes prior to your scheduled appointment for registration purposes.
Late Policy:
If you arrive fifteen (15) minutes or later to your scheduled appointment, you will need to reschedule your appointment. Our late policy is necessary to ensure that there is adequate time for your physician to complete your consult.
Cancellation Policy:
If you are unable to make a scheduled appointment, please call the appointment line at 650-723-6601 as soon as possible so we can offer the appointment time to another patient.
