How to Prepare for Your Consultation Appointment

Regardless of what type of visit you are planning, it is important to be well prepared. Some forethought and preparation will make a visit with your doctor more productive.

What to Expect from a Consultation Appointment:

A consultation appointment is an office visit with one of our sleep physicians.  During the evaluation, your doctor will review your medical history and perform a physical exam.  

You will be asked to discuss your sleep complaints as well as any other related conditions. Your physician will work with you to develop a treatment plan and may recommend an overnight sleep study.

It is always wise to prepare a list of questions and key issues you would like to discuss before your appointment to ensure all of your questions are answered.

What to do Before Your Appointment:

In order to ensure your visit goes smoothly, please:

For adults (ages 18 and older), we encourage you to complete our on-line questionnaire ( or you may bring a completed hard copy of this alternative questionnaire (PDF) to your appointment.

For children (under the age of 18), please complete our special pediatric questionnaire (PDF). We do not currently have an on-line questionnaire approved for pediatric patients.

When to Arrive:

Please arrive 15 minutes prior to your schedule appointment for registration purposes.

Late Policy

If you arrive fifteen (15) minutes or later to your scheduled appointment, you will need to reschedule your appointment.  Our late policy is necessary to ensure that there is adequate time for your physician to complete your consult.

Cancellation Policy

If you are unable to make a scheduled appointment, please call the appointment line at (650) 723-6601 as soon as possible so we can offer the appointment time to another patient.

If you have difficulty staying alert while driving please make arrangements for transportation to and from the lab.

How Much Will this Cost?

We strongly encourage you to verify your own insurance benefits.  Call the customer service number on your insurance card and ask for eligibility for benefits.  You will need the CPT code when inquiring about coverage.
