We are the two Family Practitioners as Stanford Coordinated Care starts up. We came to Stanford from Humboldt County, where we shared a family practice in the small town of Arcata for 28 years. We have been married 38 years and raised two children in Humboldt County. Although it was hard for us to leave the patients we enjoyed and knew so well, the privilege and challenge of setting up Stanford Coordinated Care and the opportunity to be closer to our children and a new grandchild, who live now in the Bay area, made us ready to move.

Alan came to his mission to help patients facing chronic conditions in an unusual way – he developed Type 1 Diabetes in 1983 after completing his medical training. “I never realized what was involved in living with a chronic condition until I was faced with one myself, and found it much harder than I had anticipated. Nothing in my medical training prepared me to take the first insulin shot, to be denied disability insurance due to a pre-existing condition, or to live with a disease that was described by experts as the ‘leading cause of adult blindness, amputations, and kidney failure in the US.’ ”

Both of our practice style evolved from that day on. We emphasized the importance of a patient’s behavior in their health, but also appreciated how hard it is to live with chronic conditions.  It takes a mixture of hope, confidence, and acquiring the knowledge to succeed. You don’t get any “days off” from your disease; nor is it curable.  But the good news is that it can be well-managed and not greatly affect how you feel or how long you live. Having social support and positive role models to follow are the keys to success.

Stanford Coordinated Care is built around those principals – care is provided by a supportive team that helps each patient become the master of their own health. Care is provided through a partnership between patients and families and their care team. Emphasis is placed on care coordination with specialists, and on gaining the skills to live successfully with whatever conditions a given patient is faced with. Above all, Stanford Coordinated Care seeks to put the patient in charge of their own health and health care. We have been having a great time putting all our ideas about medical care delivery developed over the years into practice in a new setting with lots of support, great staff and advice from Stanford employees with chronic medical conditions.

Kathan Vollrath, MD

Stanford Coordinated Care (SCC)

I am a general internist physician at Stanford Coordinated Care. I finished my internal medicine residency training at Stanford 18 years ago and then worked at Santa Clara Valley Medical Center in San Jose, CA for many years. Working there, I took care of a large number of adult patients with complex medical problems, including people with chronic pain and complex chronic illness. I also taught many medicine residents and was an Associate Program Director for the Internal Medicine Residency program.

My experience as an internist over the years brought me to the questions: "Why do we do things in medicine the way we do?" and "How can we do better?" I came to Stanford Coordinated Care for the opportunity to be part of a better way of providing primary medical care for people with chronic illnesses. I know that many people with chronic illnesses want a primary care doctor who can listen and know them and their story. They want answers to their questions and appointments when they need them, and they want their care to be coordinated and make sense. They want support in making health choices and in being in charge of their health care. Sometimes people need hope-hope that there are ways to live well with chronic illness. That is what I look forward to at Stanford Coordinated Care-creating partnerships with patients facing chronic illness that will help them meet their health goals.

I enjoy hiking and yoga when I am not getting my two children to where they need to go. I look forward to meeting you at Stanford Coordinated Care!

Deepti Randhava

Clinic Manager Stanford Coordinated Care (SCC)

My name is Deepti Randhava and I have the pleasure of being the Clinic Manager for Stanford Coordinated Care (SCC) Clinic at Stanford Hospital and Clinics. In my role, I manage members of the SCC care team while directing, planning, and overseeing the daily operations of the clinic. One of my goals is to create a patient-driven clinic environment while cultivating a safe, professional, and collaborative atmosphere in which relationships between our patients and SCC team members can flourish.

Prior to joining SCC, I worked in Performance/Process Excellence at Stanford Hospital and Clinics for 4 years. I successfully led various projects in the Emergency Department, as well as Inpatient areas, to improve processes, workflows and care experience for our patients and employees. I am well-versed in Six Sigma, Lean, and change management methodologies which allowed me implement complex projects in support of Stanford Hospital’s strategic initiatives. Before embarking in the healthcare improvement world, I was a part of the Solid Organ Transplant team at Stanford Hospital for 3 years as a Clinical Social Worker.

I hold a Master’s Degree in Clinical Social Work from the University of Toronto, Canada and a Certificate in Human Resources studies from Cornell University. I received my Bachelors in Child Development from University of Guelph in Canada. I enjoy bringing diverse perspectives from the various countries where I have lived and studied – Brazil, India, Jamaica, Canada, and Australia.

Deborah Bowes, PT

Stanford Coordinated Care (SCC)

I’m excited to be working at the Stanford Coordinated Care clinic because of the emphasis on helping people to feel as healthy and active as possible, with each person finding the path that works for them. I’m thrilled to part of a compassionate, competent, and creative team.

I’ve worked as a physical therapist in occupational health; pain programs, home health, adult day health, schools, a spinal cord and brain injury center and I maintain a private practice part-time.  I’ve been practicing and studying approaches to wellness and health that come under the heading of complementary or alternative medicine for more than 30 years. My primary specialty is the Feldenkrais Method in which I’m certified as a Feldenkrais practitioner and trainer. The Feldenkrais Method uses easy movement and awareness to improve function, reduce pain and can help one to feel more comfortable in one’s body. This method is especially useful for people who have not been helped by more traditional approaches to physical therapy.  My physical therapy degree is from Columbia University and I have a doctorate in physical therapy from Shenandoah University.

I look forward to discovering how to best meet the needs of each person who will benefit from the care at SCC and working in partnership with them.

I enjoy reading, making things by hand, sewing, meditation, Tai Chi, cooking, hanging out with family and friends, and playing the autoharp.

Susan Shughrue, BSPharm, BCACP, CDE

Ambulatory Care Clinical Pharmacist and Certified Diabetes Educator

My name is Susan Shughrue and I’m thrilled to be part of the Stanford Coordinated Care team, serving as the Clinical Pharmacist and Certified Diabetes Educator.

I started my pharmacy career in Seattle, Washington where I was first introduced to Ambulatory Care Pharmacy. This is where the Pharmacist gets to work directly with each patient and their healthcare team in a clinic setting, working to provide the best, patient-specific coordinated care. This type of patient care and clinic setting has become my passion! Since Seattle, I relocated back home to the east coast and worked in West Chester, Pennsylvania in several different positions including hospital pharmacist, drug information specialist, and clinical professor for the Philadelphia College of Pharmacy. For the past ten years, I have worked in a clinic similar to Stanford Coordinated Care, providing individual patient care with a team of healthcare professionals working together to help each patient achieve their health goals.

It was during these ten years that I became very interested and involved in the care of patients with diabetes. It was difficult for me to see patients take so many medications and have so many unanswered questions and fears. This led me to become a Certified Diabetes Educator and develop a diabetes management program where I was able to work one-on-one with patients to address their concerns about diabetes, answer their questions, set and achieve realistic goals, and work together toward a better quality of life with diabetes.

I’ve recently relocated to California with my family and have been busy exploring the area, meeting new people, and enjoying being outdoors in the beautiful California sunshine!

Coleen Travers, LCSW

My name is Coleen Travers, and it is my privilege to be the Licensed Clinical Social Worker (LCSW) on the Stanford Coordinated Care (SCC) team.

As the Clinical Social Worker, I have the opportunity to provide our patients with tools and skills to improve their confidence in dealing with their health.  Chronic illness affects every area of a person’s life (physically, emotionally, socially, spiritually, etc.), including self-esteem and relationships with others.  I will be working with our patients to educate and encourage them in the awareness of the power of their choices, which can impact their body, mind and spirit.  As an advocate I am available, as needed, to accompany our patients on visits to see their specialists or visit them if they are home-bound.  I will have the opportunity, in group or individual counseling sessions, to support people as they make changes in their life, “trying on” new behaviors or actions that will lead them to the fulfillment of their personal goals.

I have been a clinical social worker for over twenty-five years, having worked in my own counseling practice (16 years), various agencies, medical settings (hospital and outpatient/ambulatory care), as well as taught graduate and undergraduate Social Work for many years at a large Midwestern university. In addition to having experienced specialized training that addresses the needs of people with chronic illnesses, I have expertise in working with people facing depression, anxiety, grief and loss, shame, chronic pain, and trauma.

Samantha Valcourt, MS, RN, CNS

My name is Samantha Valcourt and I am the clinical nurse specialist for Stanford Coordinated Care. I was excited to become a new member of this team that is truly committed to providing patient-centered and coordinated care. Throughout my career as a registered nurse, I have seen the challenges patients face trying to navigate our complex healthcare system. I strongly believe that most people are engaged in their health but sometimes just need a little extra support so that they may feel empowered to live healthy lives. I have a great interest in preventive lifestyle changes to avoid and reduce disease complications and in the use of complementary therapies to help people live with their conditions.

I started my nursing career in Florida in 2001 and worked in a variety of settings including medical-surgical, telemetry and step-down units. Travel nursing brought me to the beautiful state of California in 2004 where I was introduced to disease management and case management to assist patients with complex health conditions requiring ongoing care. My work was solely telephone-based and so I am excited to work with people face to face again. I look forward to serving members of this community and helping to improve the transitions individuals face when they go between providers, specialists, and care settings.

When I am not at work, I enjoy the outdoors and California's scenic mountains and valleys make it an ideal setting for two of my favorite activities which are running and hiking.

Delila Coleman

Care Coordinator, Stanford Coordinated Care (SCC)

My name is Delila Coleman, I am a Certified Medical Assistant (CMA), and a member of the National Association for Health Professionals (NAHP).  I take great pride in my profession, and I count it a privilege to be part of the Stanford Coordinated Care clinic as one of their patient Care Coordinators.

I am constantly seeking creative ways to help develop a dynamic learning environment with my patients.  I strive to promote high quality patient care and continuous improvement, which is one of the reasons I am excited to be working with patients as a Care Coordinator.

I have worked in the medical field for twelve years; the last seven years have been at Stanford Hospital & Clinics Gynecology Clinic (Women’s Health).  I look forward to working with a unique patient population within Stanford Coordinated Care!

Jennifer Harpe

Care Coordinator, Stanford Coordinated Care (SCC)

I have worked in healthcare for just over 4 years and I have been with Stanford for the past year. I first worked in a private nephrology clinic for three years and really loved it. I worked very closely with people on dialysis and helped coordinate to get people ready for kidney transplants with major hospitals.

After I moved to the Peninsula, I started working at a private internal medicine office in San Francisco. I worked for a year there as the only medical assistant and was able to sharpen my skills in direct patient care. I was able to give daily injections, EKG’s, and became very familiar with obtaining prior authorizations for medications and procedures.

I joined SCC for the opportunity to work in a patient care driven environment. It is my goal for everyone to be comfortable when coming in to see the doctor and feel confident in taking care of themselves. I am happy and ready to assist in any way possible. I look forward to serving our patients as a Stanford Coordinated Care team member.

I am a native Californian and live locally in the Peninsula. I like to be outdoors and my favorite thing to do laugh; I will always have a smile on my face and try to put one yours. I love to be around people, and I look forward to meeting you!

Tammy Ojeda

Care Coordinator, Stanford Coordinated Care (SCC)

My name is Tammy Ojeda and I am a Certified Medical Assistant, Phlebotomist and Care Coordinator for the Stanford Coordinated Care (SCC) clinic. I have worked in the medical field for over 15 years with experience in multiple inpatient and outpatient clinics. I have worked in Medical Specialties, Internal Medicine and Family Practice settings as well as Wound Care and Hemodialysis. Most recently, I was a Practice Care Coordinator for Medical Specialties, Liver and Kidney Transplant as well as General Surgery and Trauma here at Stanford Hospital & Clinics. I am very passionate about our mission and believe that together as a team we can help fulfill each individual patient’s health care needs.

Outside of work I enjoy spending time with my family, listening to music, cooking, reading, and traveling.

Celina Meza

Office Assistant Stanford Coordinated Care (SCC)

My name is Celina Meza and I am the Office Assistant for the Stanford Coordinated Care team.

I joined Stanford Coordinated Care for the opportunity to spend more time with patients and help them feel more engaged in their health care. I will do my best to assist with any questions I receive. As an Office Assistant my role with Stanford Coordinated Care will be ensuring support for the appropriate operations of the office as well as interacting with patients on a daily basis. I enjoy working closely with the physicians and care coordinators to facilitate patient needs. I am driven to provide patient centered care and will be assisting new patients to Stanford as well as scheduling appointments. I provide a warm and welcome atmosphere to all patients coming to Stanford Coordinated Care.

I have worked as a Clinic Assistant for over 3 years in Stanford’s Department of Orthopedic Surgery. In my time at Stanford Hospital & Clinics, I have witnessed many transitions within the Hospital and I am excited to be a part of them. I am very proud to join the SCC team and look forward to providing excellent care to you.

I have lived in different places throughout California and Northern California is by far my favorite. I love living in the Bay Area because of the rich diversity and cultures. I often spend my weekends traveling to local farmer’s markets and festivals. I enjoy making people laugh and making them feel comfortable.
