Patient Support
The members of the transplant team collaborate to provide continuity of patient care. In addition to transplant coordinators, our team includes transplant social workers, a dietitian, a pharmacist, a diabetes educator, financial coordinators, patient care coordinators and access to an occupational therapist, physical therapist and speech therapist as needed.
The social workers collaborate with the transplant coordinators to help secure housing, transportation, and other resources, including interpreter services. The transplant social workers also provide counseling services to facilitate adjustment with the patient before and after transplantation.
Patient care coordinators a assigned to patients both pre and post transplant to provide continuity of care and assist in scheduling and coordination with the nurse coordinators.
Transplant Financial Coordinators collaborate with the clinical and patient care team to ensure appropriate coverage, optimization of benefits and approvals and counseling. This allows for the transplant patient and their family to focus on medical treatment.
A weekly diabetic support group and a monthly women's support group are available to patients.
Various other transplant support groups are available throughout the community and Internet.
Transplant candidates and patients also enjoy annual retreats and a patient-education symposium.
