During the transplant process, your blood glucose control and nutritional needs may change.
In anticipation of these changes, we have made available a blood glucose log (PDF) and food diary log (PDF) that you or your family can down load, complete and bring to your appointment so we can monitor your progress while at home.
The information you provide will help us better accommodate your needs. Please feel free to write down any questions and or concerns you may have before your appointment.
