Weekly Walk-in Appointments
The walk-in discussion sessions provide a place for patients to problem solve their diabetes-related issues. These sessions are staffed with a registered dietitian, pharmacist and diabetes educator who coordinate with the patient's transplant program.
Group leaders can help review diet, blood glucose and medication records, blood glucose meter education, nutrition consultation, and medication on an as-needed basis. Time: Mondays, 9:00 - 11:30am Location: Boswell Clinic building , First Floor
Individual Appointments
Provided on Thursday during transplant clinic appointments and / or Transplant Diabetes Endocrine appointment. These appointments offer individuals a focused time and training and are extremely helpful for patients new to diabetes or who are recently experiencing changes in their diabetes management. We will help provide timely diabetes education and support that will coordinate with your scheduled appointment.
