NOTE: New appointments are by physician referral only. Your physician's office needs to fax a letter of referral, contact information, insurance information and records to (650) 723-6002. Upon receipt of this information, the records will be reviewed by a physician and you will then be notified of the status of an appointment.
For First Appointment:
Bring any MRI or CT films. Your records should have been sent ahead of time.
Please plan to be here for one to two hours for your first appointment.
