Refer a Patient
When to Refer
- Any patient with a progressive neurological syndrome that includes cognitive or behavioral symptoms is appropriate.
- The earlier the better, as patients with Mild Cognitive Impairment may stand the most to benefit from diagnosis and treatment, especially as emerging therapies become available.
- We welcome referrals from any provider, including primary care providers, neurologists, or other health care professionals.
- We accept referrals from near and far, including from the immediate Stanford community and from around the country and internationally.
- We see patients for one-time consultations, second opinions, and for longitudinal care.
How to Refer

Before the Appointment
- Before coming it is helpful but not necessary to test thyroid function (TSH and T4) and serum B12, and to have obtained magnetic resonance brain imaging. Any prior brain imaging (films or CD) should be hand-carried to the appointment.
- Patients are required to complete a questionnaire prior to their visit.
- Family members are strongly encouraged to accompany a patient to the appointment, as a spouse, child, caregiver, or close friend may provide valuable insight into a patient's symptoms.
