Referral Information
Patients under consideration for a heart transplant will be referred to the Cardiomyopathy Center.
Generally, your physician will request an evaluation by one of the heart failure specialists. Your previous medical records will be reviewed and additional testing may be required in order to fully assess your heart condition. We also accept self-referrals.
