Are your services covered by insurance?

Depending on your insurance coverage, most often the services are covered if you have a diagnosed cholesterol or blood pressure problem, It is always  best to check with your insurance company first.

What do I need to do to prepare for my appointment?

  1. Confirm your Cardiovascular Clinic appointment day and time.  
    You should plan on being at the clinic for approximately one to two hours. Be aware that there are numerous construction projects around the medical center causing traffic delays.  Please allow ample time for travel and parking. 

  2. If instructed, please bring the results of any requested laboratory tests.

  3. Please complete the Food Frequency questionnaire which you will receive in the mail.  If so, it would be helpful if you could complete this form at your earliest convenience and return it in the enclosed envelope.  We would prefer to receive this completed form at least 7 to 10 days prior to your visit, in order for our dietitian to review this and have your analysis available at the time of your visit.

  4. Please print and bring the completed Cardiovascular Clinic Patient Questionnaire form to your appointment. 

  5. Please bring or mail any medical or personal records that you may have that relate to previous hospitalizations, cardiovascular risk factor values and/or treatment. We have access to your Stanford records.

How do I reschedule my appointment?

If you need to reschedule your appointment for any reason, please call the Cardiovascular Medicine Clinic at (650) 723-6459.  We appreciate a call at least one week prior to your new patient visit if your plans change for any reason.

How do I get to the Cardiovascular Clinic?

The Cardiovascular Clinic is located on the second floor of the Boswell Building.
