What do I need to do to prepare for my appointment?
1. Confirm your preventive cardiology appointment day and time. You should plan on being at the clinic for approximately one to two hours. Be aware that there are numerous construction projects around the medical center causing traffic delays. Please allow ample time for travel and parking.
2. If you are instructed to obtain additional laboratory test, please arrive fasting for 12 hours. This means nothing to eat or drink except for water for 12 hours prior to your blood draw.
3. Please bring the results of any relevant or requested laboratory tests. We have access to your Stanford test results.
4. Please complete the Food Frequency Questionnaire at your earliest convenience (you may also have received this in an email). In general, we prefer you complete this survey at least 7 to 10 days prior to your visit, in order for our dietitian to review this and prepare your analysis at the time of your appointment.
5. Please print and bring the completed cardiovascular clinic patient questionnaire form to your appointment.
6. Please bring or mail any medical or personal records that you may have that relate to previous hospitalizations, cardiovascular risk factor values and/or treatment. If you are already an established patient at Stanford, we have access to your Stanford records.
